  • Form 8101: Request for disaster relief

    If you cannot meet a regulatory rule due to a declared disaster, you can request disaster relief. Disaster relief is a temporary waiver of a regulatory rule. Qualified disasters are a State of Emergency declared by the Governor or a local emergency declared by the city or county. You can use this form to request temporary relief from a regulation and, if applicable, to notify us you have moved your product as the result of immediate threat related to a qualified disaster.
