  • Form 24: Data Package Cover Page and Checklist

    The licensed laboratory shall compile and generate one data package for each representative sample that the laboratory analyzes, prior to release of the certificate of analysis (COA). This form shall be signed and dated by the reviewing supervisory or management laboratory employee meeting the responsibilities and qualifications under 4 CCR section 15737.

  • Form 1602: Equity Fee Relief, Additional Owners

    Use this form if you are requesting an equity fee relief and need additional room to list qualifying equity owners.

  • Child-resistant Packaging (CRP)

    State law requires that all cannabis goods are sold in child-resistant packaging (CRP). CRP is packaging that is designed to be hard for children under five years of age to open. It is the responsibility of the licensee that packaged the cannabis good to make sure that the package meets CRP requirements.
