Licensees authorized to engage in retail activities with an adult use (A-) and medicinal use (M-) designated license, can use this form to modify the license to an A-license and a new related M-license. The related M-license may be held by a different entity than the existing license.
How to submit the form
Form 9207, along with supporting documentation, must be submitted by the designated responsible party from the email address listed for them in the license record to licensechange@cannabis.ca.gov.
